Practice Based Commissioning (PBC)

We are part of an NHS Primary Based and Secondary Care partnership with clinical leads identified from many specialities. More local area teams are being formed with integration and plans to expand further in the UK, covering wider areas.

Diagnostics UK is supporting the Practice Based Commissioning. Some surgical and medical services are being offered in the primary care setting at the GP surgery providing direct access to initial surgical workup and basic treatments.

Primary Care:

We are currently forming pathways for extended GP services, one stop diagnostics and clinical services pathway for primary care. We are participating in the procurement processes for these services undertaken by the Primary Care Trusts (PCT) and aim to provide Clinical Sessions, Governance, Audit, Training and extended GP services for the patient centred services pathway as recommended in the PBC guidelines. We are working with lead General Practitioners to help run this service and are in the process of identifying new GP practices, which can partake the clinical service pathways recommended by clinical teams. We are keen to help promote basic diagnostics and additional service, in particular as practice sizes grow. We anticipate there will be a need for on-site diagnostic services in the form of X-rays and ultrasound. We can supply, install and commission the hardware and software through our IT Partners. This enables connectivity to a datacentre for remote reporting via a secure PACS imaging viewing suite. The same setup can also be used for sending ECG's, digital images of skin or other lesions and retinal pictures for remote reporting at Diagnostics UK.

Medicine:
We are currently inviting physicians from all specialities to work across the country in Primary Care Practices. We plan to have a wide range of specialities covered, and with expansion, further specialists across the country. All our members are on the GMC & specialist registers. We aim to use our DICOM medical image reviewing and storage facilities, for other medical application including tele-dermatology and tele-cardiology (remote ECG reporting) application.

Tele-Medicine:
One of the core strengths of Diagnostics UK is access to Tele-Medicine software. Through the use of robust information systems allowing secure storage and transmission of images, and PACS software, we offer remote reporting of medicine or radiology images. DICOM images are the standard for radiological pictures and include the raw data of the image as well as patient details. This format can also be used for other medical images, including photographs of skin lesions and ECG's, providing secure DICOM transmission with our dedicated information systems providing a bespoke solution to meet NHS needs and PBC requirements.

The DICOM images data is transferred via very secure link and can be viewed on workstations or surgery IT systems / PC's. The IT and information systems can be installed by Diagnostics UK partners providing remote access to the work site or home, enabling remote reporting. We aim to provide the majority of reporting through remote links and this can be supported by a dedicated team member providing fast turn around time, enabling high quality care and meeting the needs of cold cases as well as where acute reporting is required. The remote clinical reporting is lead by various clinical specialist experts in the field, with provision for QA, Audit, double reporting and governance pathways.

Surgery:
We are currently recruiting surgeons in all sub-specialities for a wider coverage in England. All our members are on the GMC & specialist registers. We have some specialities covered, and as we add more wider area and subspeciality coverage, we will need further specialists across the country. Since 2006, our team have been providing surgical services, with a local anaesthetic list in a Primary Care Practice in Nuneaton, run on a Practice Based Commissioning (PBC) basis.

We aim to use our DICOM medical image reviewing and storage facilites that are currently in use for tele-radiology reporting for tele-ophthalamology applications.
